Student Request and Reporting Forms
New York Institute of Technology’s College of Osteopathic Medicine is committed to ensuring that all students have the same opportunity to succeed academically and to promote health and safety. The Office of Student Administration reviews all cases involving student conduct, student concerns and requests for student support within the College of Osteopathic Medicine. This site provides online forms for reporting incidents involving students. Our staff is available to explain procedures and talk to you about your concerns about reporting an incident or situation.
Report Academic Misconduct
In an academic community, faculty members play the most important role in maintaining academic standards. When an act of academic dishonesty is observed or suspected, a process is in place to uniformly address such events. Community members who suspect that academic misconduct has occurred should complete this form.
Report Student Misconduct
All NYITCOM students and student organizations are expected to abide by expectations of responsible behavior as established under the Student Code of Conduct. Students, faculty, staff and other persons are encouraged to report concerning behavior that appears to disrupt our shared learning environment. Reports made here are for non-emergency incidents only. Please review the Student Code of Conduct if you have any questions.
Report Violations of Title IX
Reports of sexual misconduct, including sexual harassment, sexual assault, gender-based harassment, intimate partner violence, sexual exploitation and stalking are investigated by the Title IX Coordinator. Individuals who have experienced sexual misconduct have the ability to submit a report anonymously if they choose.
Request Help to Resolve a Conflict
Use this form to request an appointment with a member of the Student Conduct team to assist in resolving a conflict. Please provide as much information as possible and we will reach out with possible appointment times.
Things to keep in mind when working with Student Conduct in Conflict Resolution:
- Participation is voluntary.
- The Conflict Resolution Process is separate from the Student Conduct Process.
- Our office acts as a facilitator, not a decision maker.
Report a Concern for a Student
Medical school can sometimes be overwhelming. NYITCOM assists the university community to help assess and find solutions for managing distressed students that may be experiencing unexpected life events, personal crises, mental health struggles, and academic difficulties that can interfere with their academic success. Use this form to report concerning behavior and to request a consultation with our staff.
Request for Emergency Funds
Student may find themselves in an emergency situation where they need immediate financial support. NYITCOM offers limited emergency grants to assist students facing these challenging situations. Support is available if a student finds themselves in circumstances such as: natural disasters, local or national emergency situations, medical emergencies, family illness or another emergency. Students who find themselves in need of such support, should complete this form.
Student COVID-19 Immunization Submission or Waiver Requests
Students, both new to NYITCOM and currently enrolled students may be required to prove that they are immunized against COVID-19 or submit a request for a medical or religious exemption. Students wishing to submit proof of COVID-19 vaccination, should complete this form.
Any student who wishes to file an application for a waiver of the COVID-19 vaccination for medical reasons, should complete this form.
